Cancellation Policy

Waiver and Release of Liability I, the undersigned, hereby acknowledge and agree to the following: Assumption of Risk: I understand that participation in martial arts at First to Strike Freestyle Martial Arts involves physical contact and exertion that carries an inherent risk of injury, including but not limited to strains, sprains, bruises, broken bones, or more serious injuries. I voluntarily assume all such risks. Medical Fitness: I certify that I (or my child, if under 18) am physically fit and have no medical condition that would prevent participation in martial arts activities. I understand it is my responsibility to consult with a physician regarding any concerns. Release of Liability: I release and hold harmless First to Strike Freestyle Martial Arts, its owners, instructors, employees, and agents from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, or injury that may be sustained while participating in any class or related activity, whether caused by the negligence of the releasees or otherwise. Consent to Treatment: In the event of an emergency, I authorize the staff of First to Strike Freestyle Martial Arts to obtain medical treatment for me (or my child) if necessary, and I assume responsibility for any medical bills incurred. Photography & Media Consent: You give permission for First to Strike Freestyle Martial Arts to use photos or videos taken during training for promotional purposes.
